Patient Information

Everything new and returning patients need to know about scheduling, insurance, payments, and preparing for your first visit.

New Patients

Welcome to Brightside Family Dental. Here is what to expect from your first visit with us.

Before Your Appointment

New patient paperwork can be completed online through the link we send after scheduling. We recommend completing this at least 24 hours in advance so our team can verify your insurance and prepare for your arrival.

Please bring to your first appointment:

  • A valid government-issued photo ID
  • Your dental insurance card (if applicable)
  • A list of any current medications and dosages
  • Any dental records or X-rays from your previous dentist (these can also be transferred electronically)

Your First Visit

New patient appointments are typically 75–90 minutes to allow time for:

  1. Comprehensive oral exam including X-rays
  2. Periodontal assessment
  3. Oral cancer screening
  4. Cleaning (if clinically appropriate on the same day)
  5. Treatment discussion and planning

After your first visit, you will leave with a written treatment plan summary including any recommended follow-up, estimated costs, and your estimated insurance coverage.

Insurance

We are in-network providers for most major dental insurance plans, including Delta Dental, Cigna, Aetna, MetLife, United Concordia, and Guardian.

For plans we are not directly contracted with, we will file claims on your behalf as a courtesy and help you maximize your out-of-network benefits.

Important notes:

  • We verify your insurance before your appointment and provide an estimated co-pay in advance
  • Coverage varies significantly by plan — we review your specific benefits before treatment, not after
  • Annual maximums and deductibles reset January 1 for most plans; we can help you utilize remaining benefits before year-end

No Insurance?

Ask us about our in-house Brightside Care Plan — a simple annual membership that covers preventive services and provides a flat discount on other treatment. No deductibles, no claim forms, no annual maximum.

Payment Options

We accept the following forms of payment:

  • All major credit cards (Visa, Mastercard, American Express, Discover)
  • Cash and personal checks
  • FSA and HSA cards
  • CareCredit and Sunbit financing (0% interest promotional periods available)

For larger treatment plans, we are happy to stage treatment across benefit years to minimize out-of-pocket costs.

Scheduling Policy

Cancellations: We ask for 48 hours’ notice for cancellations or reschedules. Late cancellations may incur a $75 fee for unfilled appointment time.

Running late? Call ahead. We will do our best to accommodate you, though appointment length may need to be adjusted for same-day care.

Emergencies: Call our main line. We hold same-day availability for acute dental pain, trauma, and infections.

Financial Policy

Treatment cost estimates are provided before work begins. We do not proceed without your informed consent and understanding of what insurance is expected to cover versus what is your responsibility.

For treatment exceeding $1,000 in patient-responsibility costs, we offer split-payment arrangements at no additional cost.
